School Health Questionnaire
Parents and Carers are invited to complete a health questionnaire for their child in reception year. The aim of the questionnaire is to identify any support that your child may need. The school nurse team will review the health information and may contact you to discuss your child’s health needs further. If you require any further information, need help completing the questionnaire or have any queries, please contact the school nursing service.
